ZERO TOLERANCE FOR ABUSE TO OUR STAFF 

A zero tolerance policy towards violence and aggression is expected throughout services. No member of staff should be subjected to violent, threatening and abusive behaviour.

CANCELLED AND MISSED APPOINTMENTS

We require at least 1 full business day's notice if you cancel/change an appointment.  All no shows and same day cancellations/changes are subject to a no-show fee:

• No show fee for new consult: $150
• No show fee for follow-ups: $100

PROVINCIAL HEALTH CARE CARD

For first visits, bring a photo ID and a health care card with your personal health number (PHN) on it.  You will be charged for your visit should you not have any active provincial health coverage.

CONFIDENTIALITY

All staff at Peak Medical are informed on issues of confidentiality, and exercise care in ensuring patient confidentiality. All patients have the right to confidentiality. A patient’s medical information cannot be released to family members or friends without the expressed verbal or written consent of the patient. Adolescent minors have the right to privacy for clinical visits. As such, parents accompanying minors may be asked to leave the examination room for at least a portion of the clinical visit. This is by no means a reflection of concerns about the parent-child relationship. We are unable to release information to a third party (ie. law offices, insurance companies), without signed authorization from a patient or, if applicable, his or her guardian.

PRESCRIPTION REFILLS & RENEWALS

We do not provide urgent prescription renewals. Please expect up to 5-10 business day turnaround times for prescription refill requests to our office. Your family physician or nurse practitioner will also be able to assist you. For urgent refills, please speak with your pharmacist, and they may be able to provide an emergency refill of your prescription, including medications for chronic conditions dependent on the situation.

JOINT INJECTIONS

Cortisone injections may be provided by our rheumatologists.  The procedures are not charged to the patient but cortisone medications used will be (charged at $15 per cortisone vial; usually requires 1-2 vials per area injected).  A receipt will be provided if you have insurance that covers the costs of your medications.

USE OF RECORDING DEVICES

It is prohibited to take pictures, videos or audio recordings in public areas such as the waiting room to protect the privacy of other patients.

If you would like to record your appointment with the medical staff, please ask for their consent in advance.

ANIMAL POLICY

Only service animals with proof of certification are permitted in the clinic as our clinic serves patients with potential severe allergies to animals.
